The DVA referral system

A DVA Referral initiates the Allied Health Treatment Cycle, an approach designed for DVA clients to be efficiently referred to allied health providers. This cycle improves coordination and communication between general practitioners (GPs), allied health professionals, and clients.

The DVA referral process connects veterans with the best possible allied health care. As a DVA client, your GP acts as your care coordinator, working collaboratively to ensure you receive the most appropriate treatment.

Step one

Obtain a DVA Referral from your GP

Your GP will need to complete DVA Referral form D0904. You can download this form here.

Step Two

Submit your details

Complete our DVA Referral patient intake form below and attach the completed DVA Referral form D0904 from you GP in the relevant section. If you do not have a completed D0904 form to attach, you will be prompted to provide this prior to your appointment.
